Student Organization Forms

  • Administrative Request Form*
    Use this form to change your organization's type from Chartered to Recognized or vice versa. This form is also used to changed your organization's official name.
  • New Organization Application
    This is for starting a new student organization that does not currently exist at Duke and/or have a presence on Duke Groups.
  • Request SOFC/DSG Status
    This form should be used for existing groups that are sponsored by a department/school/college on campus and want SOFC/DSG status.
  • Contract Request Form*
    Use this form to initiate a contract for speakers, performers, etc. that your organization is bringing. This form must be submitted BEFORE your event takes place.
  • Payment Request Form*
    Use this form to submit invoices for payment. This form is also used for reimbursements.

*These forms are only available to groups established under Duke Student Government (DSG/SOFC).

  • Name Change Request*
    This form is also used to changed your organization's official name.
  • New Organization Application

    This form should be used for existing groups that are sponsored by a department/school/college on campus and want GPSG status. If your group does not have already have a presence on Duke Groups, please use the New Organization Application link.

Fraternity & Sorority Forms

Fraternities and sororities are opportunities for students to meet like-minded peers and connect with one another in values-based organizations. Fraternities and sororities provide community within national and international networks as members join chapters at Duke. 

Students interested in joining a fraternity or sorority are encouraged to explore all the chapters at Duke through council information, national organizations’ websites, and activities and events on DukeGroups. Questions? Contact us at leadership@duke.edu.

The Duke Greek Community Standards (DGCS) is the baseline expectation that we have for all organizations to be recognized by Duke University.  

Submissions are due at the end of each semester. Results are publicly available on the Community Scorecard.

DGCS Packet: All information pertaining to the Duke Greek Community Standards. The packet outlines all of the criteria that chapters are expected to meet to be recognized by Duke University. Additionally, Chapters may submit additional information to become eligible for the annual "Excellence Awards."
