- Create a Contract*
Use this form determine which contract option works best for you. This will allow you to create a contract and get it signed by Duke. - Payment Request Form*
Use this form to submit invoices for payment. This form is also used for reimbursements. - Name Change Request
Use this form to request a name change for your student group. This will require the approval of the group's President and Advisor.

Fraternity & Sorority Forms
Fraternities and sororities are opportunities for students to meet like-minded peers and connect with one another in values-based organizations. Fraternities and sororities provide community within national and international networks as members join chapters at Duke.
Students interested in joining a fraternity or sorority are encouraged to explore all the chapters at Duke through council information, national organizations’ websites, and activities and events on DukeGroups. Questions? Contact us at leadership@duke.edu.
Forms & Guidelines
Duke chapters planning intake activities in any given semester must:
- Submit the Intent for Membership Intake Form on DukeGroups at least 2 weeks (14 days) before their first formal interest meeting or rush event.
- Register all recruitment and intake events on DukeGroups
As part of the recruitment and intake process, students interested in joining a fraternity or sorority must:
- Complete the Grade Release Permission Form on DukeGroups for University staff to verify their eligibility to join
- Meet with Student Involvement & Leadership staff for the university-provided Embark Values Clarification program during the first two weeks of new member intake activities.
- Complete the Verification of Members Form on DukeGroups after being initiated
The Duke Greek Community Standards (DGCS) is the baseline expectation that we have for all organizations to be recognized by Duke University.
Submissions are due at the end of each semester. Results are publicly available on the Community Scorecard.

DGCS Packet: All information pertaining to the Duke Greek Community Standards. The packet outlines all of the criteria that chapters are expected to meet to be recognized by Duke University. Additionally, Chapters may submit additional information to become eligible for the annual "Excellence Awards."
