Enrolling in or Waiving SMIP

Duke University students will be automatically enrolled in the SMIP each year they are enrolled at Duke. Non-international students who have current coverage with another insurance plan that meets specific criteria may waive or opt out of the Duke medical plan.  International students holding an F1 or J1 visa are required to participate in the Duke SMIP and are not able to opt out of the medical plan.

Enrolling in the Duke SMIP

Students who are required to participate in the SMIP by either enrolling in the plan or providing proof of alternate coverage may opt to enroll in the Duke plan via the online Gallagher student insurance portal.  This is the university’s partner in administering this plan. 

If you plan to enroll in the plan, you do not need to take this action–you will be automatically enrolled at the end of the waiver period.  This is an option for those students who wish to no longer receive reminders to take action prior to the end of the waiver period. 

Waiving SMIP

Students who already have medical insurance may be able to opt out of the Duke SMIP by submitting a waiver request.  As we want to ensure students have adequate, in network coverage here at Duke, your existing insurance must meet specific criteria in order for that waiver request to be approved.

The criteria for students to have a waiver request approved are:

  • Student must not be attending Duke on an F1/J1 visa
  • Plan must be in compliance with the Affordable Care Act (ACA Compliant)
  • The claims administrator of the plan is based in the United States, has a U.S. telephone number and address for submission of claims, and the insurance policy has not been issued outside the U.S.
  • The policy is not a traveling policy.
  • The plan provides unlimited emergency and non-emergency routine health care and mental health benefits in the Durham, NC area, including at Duke Medicine. (In order to meet these criteria, your plan must provide in network benefits at Duke Health facilities for the following services: Primary Care Doctors office visits, Specialty office visits, Preventative care office visits, Lab services, X-rays, and imaging)
  • The plan provides inpatient and outpatient mental health care (with at least 30 visits per year) with chemical dependency benefits that are comparable to the coverage provided by the Duke SMIP.
  • The plan provides unlimited coverage for prescription medication.

NOTE: these criteria apply to all plan types without exception, including Medicaid, Medicare, TRICARE/military, HMOs (including Kaiser, WHA, and others), U.S. federal or state exchange plans, and all employee-sponsored and individual plans.  Out-of-state Medicaid and state Children’s Health Insurance Plans, HMOs, and Kaiser Permanente insurance plans do not cover non-emergency care in Durham and will not qualify for a waiver. 

Steps to Submit a Waiver Request

All students must submit their waiver annually during the fall open enrollment and waiver period, listed at the top of this page.  Students who are incoming students during the spring semester must complete any waivers during the spring open enrollment and waiver period. Waivers will not be accepted after the listed waiver period end date.

If your plan meets the criteria above, follow these steps to submit a waiver request:

  1. Log in through the secure Gallagher Student portal.  This is the university’s partner in administering this plan.
  2. Follow the instructions to submit information about your current insurance plan.
  3. You will receive an email directly from Gallagher with the status of your waiver request, typically within 3 to 5 days.
  4. If your waiver is approved after charges have been added to your student account but before the end of the waiver period, the charge will be reversed in 7 – 10 business days.
  5. Once you receive approval, which will be sent through email confirmation, you’re all set!  This action is required once per academic year.
